Our Mission

Prima Care Health Services, Inc. addresses the Behavioral Mental Health Challenges of Adults, Adolescents, and Children within their families and communities.

Our approach encourages full participation of the consumers in their individual Rehabilitation Plans (IRP).

Prima Care Health Services (PRP) was incorporated on August 18, 2010, to uplift the mind of our clients to integrate or reintegrate them into their communities by giving them tangible value systems based on approaches and concepts of communal activities of everyday living.

This will allow our clients to meet their full potential and open doors to opportunities to meet and fulfill their dreams that may not have been attainable without rehabilitation services or behavioral modification services rendered to alleviate and gain control of the symptoms of their illnesses.

We hope to become the front runner of psychiatric rehabilitation services and act as a bridge to reconnect not just a support for our clients but as a healthcare provider from whom they will receive services.

Scope

Prima Care Health Services is a Psychiatric Rehabilitation Program for minors, adolescents, and adults. It will be a goal-oriented, outcome-focused program that works within a reasonable timeframe. Intervention strategies are jointly designed with families and individuals to reduce maladaptive behaviors to restore and strengthen specific age-appropriate skills.
These will help the enrolled individuals function at their highest potential, including individual independent living, behavioral awareness competency, and social skills training in groups or individually at the agency program site or off-site locations.

Upon eligibility determination of enrolled individuals, a licensed mental health professional shall undertake an initial assessment of each individual with the active collaboration of families/guardians and significant others. Outcomes of the professional evaluations will determine the options for rehabilitation. Consumers, families, guardians, and significant others will have an input based on individual choices and preferences. Approval to give services based on intervention strategies that have evolved as well as the goals jointly developed shall be obtained from the Public Mental Health Systems.

The challenges of rehabilitation require individualized attention. Consequently, the agency shall have a staff ratio of 1:6 for minors and a staff ratio of 1:10 for adults. Each staff encounter with a client is programmed to run for one hour and at a minimum of 45 minutes.

Psychiatric Rehabilitation Program Facts:

Paid by Medicaid. Weekly Visits by Psychiatric Rehabilitation Counselors. Services are provided on-site and off-site (Home and community).
